New Patient Initial Visit

Congratulations for making the decision to improve your health. To prepare for your first visit, we require that you fill out the New Paitent Information Form. It is important that you complete this form and get it back to us BEFORE your appointment. This allows our doctors to familiarize themselves with your case and do any needed research before your visit.

There are detailed instructions within the form explaining how to submit the form to us. Basically, you'll open the form, complete it, save it to your computer and then attach it to an email and submit it to patientservices@ahicatlanta.com or you can Fax it to 770-937-9226.
